- Formsite.com hosts Internet forms for clients. We provide the environment in which personal data is collected. Personal data may include Name, Company, Address, Phone Number as well as other personal information. Note that we have no control over the ways in which our clients use personal data, but we provide the tools with which to protect data. Data is disclosed to third parties as detailed in our Privacy Policy, including in situations in which the client configures our service to use external processing. Formsite collects personal data from account holders of the service, such as name, address, e-mail address, company name, IP address and credit card information. This information is used to identify users of Formsite and is shared with Formsite's billing provider to process credit card and invoice payments.
