- We collect the following personal information from patients on our website forms. First and Last name, Email address and Phone number. This is used for the sole purpose of scheduling appointments or returning contact requests. We collect data from the device and application used to access websites, such as Internet protocol (IP) addresses, operating system, device type, browser type and timestamps. Usage data. We collect usage data that may include which webpages are visited, what is clicked on, when those actions were performed. Emails sent by us collect information about who opened those emails and clicked on links in them. We do this to measure the performance of our email messaging and to monitor and improve email deliverability and open rates. We do not disclose any personal data to third parties.
